Abstract Typed letter from C.D. Spivak in Denver, Colorado to Mr. L. Ginsburg, secretary of Brass City Lodge #240 O.B.A. in Waterbury, Connecticut informs that Mrs. Lillie Robinson, a member of Brass City Lodge, has been invited to enter the Sanatorium of JCRS. Letter is unsigned but "Secretary" is typed at the bottom.

Information on the form includes her age, birth place, residence, occupation, marital status, children's ages, membership to beneficials societies or lodges, and nearest relatives. At the bottom of the page, under "Recommendation," is one name and address. The name is illegible but the address reads "1471 Knox Court."
