Abstract Typed letter from C.D. Spivak to N.P. Levin. Spivak asks Levin to obtain the name of the secretary and address from the lodge that Louis Cohen belongs to. Spivak tells Levin that Cohen neglected to fill that information out on his application. The name and address is written in pen on the bottom of the letter.
