Abstract Typed letter on The Jewish Consumptives' Relief Society letterhead from C.D Spivak in Denver, Colorado, to Mr. Nathan Goldfield in Denver, Colorado, inviting him to the Sanitorium of the J.C.R.S and requests that he present the letter to the Superintendent of the Sanitorium within two days. Letter states that his stay at the Sanitorium will expire two months after the date of admission. Letter is unsigned but "Secretary" is typed at the bottom.

Abstract Application for admittance to JCRS filled out by hand in ink for Nathan Goldfield. Information on the form includes his age, birth place, occupation, residence, marital status, children's ages, nearest relatives, and signed by Nathan Goldfield. The back of the form has section entitled "Report of Medical Examiner," much of which is illegible, legible writing states that a test should be given and signed by C.D Spivak.
