Lewin, acting supervisor of the JCRS Sanatorium in Edgewater, Colorado informs that he has enclosed with this letter a certificate of identity for Mr. Alex Vendig who died at the sanatorium on June 28. Requests that Lewin "kindly fill out same." Letter is unsigned but "Secretary" is typed at the bottom.

Lists effects as $3.99 in cash, broken gold-filled watch, cuff buttons (gold). Letter is signed by J.B. Fish, superintendent.
