Spivak in Denver, Colorado, the letter reads "Dec 2, 1909, CD Spivak Secy., please pay bearer 75 [stags years 1st]. Mrs. Minnie Rappapart New York refused payment. Respectuflly, [postal telegraph]".

Information on the form includes his age, birth place, occupation, residence, marital status, children's ages, nearest relatives, and signed by Hyman Bilsky. At the bottom of the page, under "Recommednation," has been left blank. The back of the form has a section entitled "Report of Medical Examiner," which is dated August 22, 1908, stating that the examination was made one day prior to filling out the application...
