Overview Application form of Benjamin Rothman for admission as a patient to the Jewish Consumptives' Relief Society. He was age 28 at the time of the application. He was born in Austria and immigrated to the United States in 1902. He lived in New York when he contracted tuberculosis. He had been sick for 3 years upon arrival to Denver, Colorado. Overview Typed letter from C.D. Spivak to E. Rothman. Spivak enclosed a membership application as Rothman requested to enroll as a volunteer at JCRS. Spivak also says that it is a great deal of pride and pleasure to know that Benjamin Rothman is speaking so highly about the sanatorium. He includes a postscript at the bottom of the letter that says that he will send Rothman a copy of the sixth annual report in a separate letter.
