Overview Application form of Mayer Bozner for admission as a patient to the Jewish Consumptives' Relief Society. He was age 54 at the time of the application. He was born in Russia and immigrated to the United States in 1903. He lived in Minneapolis, Minnesota when he contracted tuberculosis. He had been sick for 5 weeks upon arrival to Denver, Colorado. He also worked as a tailor. The verso of the application states he was admitted on May 26, 1910 and left on August 12, 1910.
