Typed letter from C.D. Spivak to H. Masliansky informing him that Nathan Leib was invited to the sanatorium as an emergency patient on May 16th, 1910.

Typed letter from C.D. Spivak to Mr. Israel Leib informing him that his son, Nathan Leib was invited for admission to the Jewish Consumptives' Relief Society as an emergency case. Spivak trusts that Leib's stay at JCRS will be beneficial for his health. He signs the letter "Secretary" at the bottom.

Typed letter from C.D. Spivak to N.P. Levin asking him to admit Nathan Leib as an emergency patient. Spivak tells Levin that he received a note from Dr. E. Friedman which stated that Leib had a hemorrhage and was not receiving any attention at this boarding house. Spivak signs the letter “Secretary” at the bottom.

Handwritten note from Dr. N.P. Levin to C.D. Spivak informing him that Nathan Leib died that day at 7a.m. Levin mentions that Leib did not leave any valuables at the time of his death.
Nathan Leib's Application for Admission to JCRS, 1910 May 16
Application form of Nathan Leib for admission as a patient to the Jewish Consumptives' Relief Society. He was age 24 at the time of the application. He was born in Austria and immigrated to the United States in 1905. He lived in New York when he contracted tuberculosis. He had been sick for 3 years upon arrival to Denver, Colorado. He also worked as a leather cutter. The verso of the application states he was admitted as an emergency case on May 16, 1910 and died on June 15, 1910.

Handwritten note from Dr. Friedman to C.D. Spivak. The note expresses that Nathan Leib had a hemorrhage and needs care. Friedman asks Spivak to admit him into the sanatorium as an emergency case.
