Application for admittance to JCRS filled out by hand in ink by Phillip Rittenberg. Information on the form includes his age, birth place, occupation, residence, marital status, children's ages, nearest relatives, and signed by Phillip Rittenberg. At the bottom of the page, under "Recommendations," are two names and addresses. E. Goldbloom, 2229 Arapahoe St., and A. Zederbaum, 1429 Stout St. On the back, on the left hand side reads "Admission and Dismission Committee," which is blank. The middle section of the back is a form filled out by hand in ink. It reads "Application No.811, Case No. 571, Application of Phillip Rittenberg, Received June 15, 1908, Accepted July 19, 1908, Admitted July 20, 1908, Discharged November 26, 1908."On the right hand side, dated June 19th, 1908 reads "Report of Medical Examiner" and includes patient notes written by A. Zederbaum stating that patient is in second stage of pulmonary tuberculosis. Additional notes are written but illegible.

Scope and Contents
From the Series: This series of patient records includes medical reports and histories, statistical data of patients, x-rays, and death certificates. The patient files include applications, correspondence and some may also contain photographs and personal items.
Inscriptions and Marks
On the front of the top page "JUN 15 1908" is stamped. Written in ink on the middle of the back page to the left of "Application Of" reads "Second."
