From the Series: This series of patient records includes medical reports and histories, statistical data of patients, x-rays, and death certificates. The patient files include applications, correspondence and some may also contain photographs and personal items.
Inscriptions and Marks
Stamp in black on top right corner is largely missing. Written in pencil to the left of the stamp reads "812." Below this in pencil reads "JCRS." Stamped to the left of the address "REMOVED 101" is stamped in red. Part of the address "Metropolitan Bldg." is crossed out in pencil.
