Harry Feldman's Application for Admittance to JCRS, 1908 July 6
Application for admittance to JCRS filled out by hand in ink for Harry Feldman. Information on the form includes his age, birth place, occupation, residence, marital status, children's ages, nearest relatives, and signed by Harry Feldman. At the bottom of the page under "Recommendation," are two names and addresses. Samuel Miller, 1378 King St., and George G. Klein, 411 25th St. The back of the form has section entitled "Report of Medical Examiner," which is dated "July 10 1908" in ink and is partially illegible. Remarks reads "Rough rest upper 1/4 posterior with fine [compilant] [illegible] [thick] are not constant but can envariably be elected by act of cough." Remarks are signed "C.D. Spivak." The middle section of the back is a form filled out by hand in ink. It reads "Application No. 833 Case No. 596, Application of, Harry Feldman, Address 1522 Julian St., Denver, Received July 6, 1908, Accepted Aug 22, 1908, Admitted Aug 24, 1908, Left Sept, 16, 1908." The bottom of the page reads "Admission and Dismission Commitee" which is blank.
