Overview

Second application for admittance to the Denver JCRS filled out in ink by patient #973 Meyer Buxbaum dated "SEP 15 1906" [sic]. The application includes patient's name, age, birth place, occupation, residential information, medical information, marital status, number of children and their ages, means of support, and nearest relatives of friends "Miss E. Buxbaum 215 E. 4th ST. New York City," and the form is signed Meyer Buxbaum [sic]. The bottom of the page reads, "RECOMMENDATION," and has been left blank. The back of the page has two sections titled "REPORT OF MEDICAL ADVISORY BOARD" and "REPORT OF EXECUTIVE COMMITTEE," which have both been left blank. Under the two blank sections of the back there is another section and it reads, "NO 311 Second APPLICATION OF Meyer Buxbaum Received September 15/06 Date of Admission February 26 1907 Date of Discharge March 31 1908" [sic].

Stamped in blue on the front upper-right area of the application reads, "RECEIVED__SEP 15 1906__ ACCEPTED___ADMITTED___DISCHARGED___LEFT___DIED___" [sic].
