Overview

JCRS Application for Admittance filled out in ink for Patient #970 Abraham Finberg "[t]o the Executive Committee of the Jewish Consumptives' Relief Society, Denver, Colorado" dated "November 20th 1908" [sic]. The application includes patient's name, age, place of birth, citizenship, occupation, residential information, medical information, marital status, number and ages of children, and nearest relatives or friends; "Bertha Finberg (Mother) 10 Fayston St Roxbury Mass. L Goldstein (brother-in-law) [same address]," and the application is signed A Finberg. The bottom of the application reads, "RECOMMENDATION... Name A. Kravitz Address 335 17th Ave. Denver Colo. Name Dainel E. Hepner Address 337 - 17th Ave" [sic]. The back of the application reads, "REPORT OF MEDICAL EXAMINEER Date Nov 21...Stage II Remarks involvement of rt upper [and] left upperlobes - [illegible]," and this section is signed "Hilkowitz" [sic]. The back-middle section of the application reads, "Application No. 970 Case No. 697 APPLICATION OF Abraham Finberg...Received Nov. 20 1908, Accepted Feb. 23 1909, Admitted Feb. 23 1909, Died Apr. 7 1909" [sic]. The bottom of the back of the application reads, "ADMISSION AND DESMISSION COMMITTEE," and has been left blank.
