Overview

JCRS Application for Admittance filled out in ink for Patient #963 S. Pitchinoc "[t]o the Executive Committee of the Jewish Consumptives' Relief Society, Denver, Colorado" dated "Nov 12, 1908" [sic]. The application includes patient's name, gender, age, place of birth, citizenship, occupation, residential information, medical information, marital status, number and ages of children, and nearest relatives or friends; "Mrs Rosie Green [Greene] 408 Van Sekland [Sicklen] ave E. New York [and] Mr. Goldberg 1383 Hazel Court," and the application is signed Mrs Sarah Bitchinoc [Pitchinoc]. The bottom third of the application reads, "RECOMMENDATION" including two names and addresses as followed: "A. Susman 2626 Walnut St. [and] Louis Schapiro 2709 W Colfax ave" [sic]. The back of the application reads, "REPORT OF MEDICAL EXAMINEER Date Nov 19 1908...Stage III Remarks Advanced type - Complete involvement [illegible]" and this section is signed "Hillkowitz" [Hilkowitz] [sic]. The back-middle section of the application reads, "Application No. 963 Case No. 668 APPLICATION OF Sarah Pitchinoc Address 1383 Hazel Court Denver, Colo, Received Nov. 12, 1908, Accepted Dec, 9 1908, Admitted Dec. 10, 1908, Died Dec. 28 1908" [sic].
