JCRS Application for Admittance filled out in ink for Patient #962 A. London "[t]o the Executive Committee of the Jewish Consumptives' Relief Society, Denver, Colorado" dated "Nov 21, 1908" [sic]. The application includes patient's name, age, place of birth, citizenship, occupation, residential information, medical information, marital status, number and ages of children [none], and nearest relatives or friends; "C. N. Meltsner 43 W. 117th St NY brother in law [and] K. London 266 Madison St. [illegible] New York (uncle)," and the application is signed Aaron London. The bottom third of the application reads, "RECOMMENDATION," and this section is blank. The back of the application reads, "REPORT OF MEDICAL EXAMINER Date Nov 21 1908...Stage II Remarks Rt lung infiltrated down to base" and this section is signed "Hillkowitz" [Hilkowitz] [sic]. The back-middle section of the application reads, "Application No. 962 Case No. 659 APPLICATION OF Aaron London Address 1721 Park Ave Denver, Colo, Received Nov. 21, 1908, Accepted Nov. 23, 1908, Admitted Nov. 25, 1908, Discharged June 15 1909" [sic].
