Application for admittance to JCRS filled out by hand in ink for Barnet Jacobs. Information on the form includes his age, birth place, occupation, residence, marital status, children's ages, nearest relatives, and signed by Barnet Jacobs. The back of the form has section entitled "Report of Medical Examiner," next to stage reads "Condensation, No moisture." Next to "Remarks" reads "Patient has arrested functioning of Right apex-No fever-[Threat unstable]" and is signed Emanuel Friedman. The middle section of the back is a form filled out by hand in ink. It reads "Application No. 847 Case No. 583, Application of, Barnet Jacobs, 2929 W. 16th Ave., Denver, Received July 20, 1908, Accepted July 29, 1908, Admitted July 31, 1908, Died Aug. 29, 1908" The bottom of the page reads "Admission and Dismission Commitee" which is blank.

Written in pencil on the back middle of the page, under "Remarks" reads "7/24/08 wrote to us, Lorber to investigate." Below note is a stamp which reads "EMERGENCY."
