Overview

The Second JCRS Application for Admittance filled out in ink for Patient #958 I. Stoller "[t]o the Executive Committee of the Jewish Consumptives' Relief Society, Denver, Colorado" dated "Nov 7, 1908" [sic]. The application includes patient's name, sex, age, place of birth, citizenship, occupation, residential information, medical information, marital status, number and ages of children, and nearest relatives or friends; "Wife - Sophie Stoller 490 Newdover Ave. Bronx...Cousin M. Philips 8 No. Front St. Baltimore Md." and the application is signed I Stoller. The bottom third of the application reads, "RECOMMENDATION," and this section lists two names and addresses: "Nathan Cohen 2724 W. Colfax Av [and] Max Potahnik 2719 W Colfax." The back of the application reads, "REPORT OF MEDICAL EXAMINER Date Nov 7 1908...Stage II Remarks [illegible] type" and this section is signed "Hillkowitz" [Hilkowitz] [sic]. The back-middle section of the application reads, "Application No. 958 Case No. 670 2nd APPLICATION OF Isaac Stoller Address 2724 W. Colfax Ave. Received Nov. 7th 1908, Accepted Dec. 10, 1908, Admitted Dec. 11, 1908, Died Feb 6 1909" [sic].
