Overview

JCRS Application for Admittance filled out in ink for Patient #958 I. Stoller "[t]o the Executive Committee of the Jewish Consumptives' Relief Society, Denver, Colorado" dated "Jan 21, 1908" [sic]. The application includes patient's name, sex, age, place of birth, occupation, residential information, medical information, marital status, number and ages of children, and nearest relatives or friends; "Wife Sophie Stoller 238 Clinton St New York - Cousin [M.] Philips 8 North Front Baltimore Md" and the application is signed I Stoller. The bottom third of the application reads, "RECOMMENDATION," and this section lists two names and addresses: "Louis Schapiro 2709 W. Colfax Ave [and] [illegible name] 2626 W Colfaxave." The back of the application reads, "REPORT OF MEDICAL EXAMINER" and has been left blank. The back-middle section of the application reads, "Application No. [684] [stamped in blue] Case No. 463 APPLICATION OF Isaac Stoller Address 1738 Curtis St. Received [JAN 22 1908] [stamped in purple], Accepted [Feb] 10, 1908, Admitted [Feb] 12 1908, Discharged March 22 1908" [sic].
