Overview

JCRS Application for Admittance filled out in ink for Patient #957 E. Sayetta "[t]o the Executive Committee of the Jewish Consumptives' Relief Society, Denver, Colorado" dated "Nov 9th 1908" [sic]. The application includes patient's name, sex, age, occupation, residential information, medical information, marital status, number and ages of children, and nearest relatives or friends; "Sister Fannie Winsberg 55 Loyal Pl. Chicago Husband +children Louis Sayetta 1675 Elliot St." and the application is signed Ester Sayetta. The bottom third of the application reads, "RECOMMENDATION," and this section lists two names and addresses: "Nathan Block 2933 Welton [and] Louis Schapiro 2709 W Colfax Ave." The back of the application reads, "REPORT OF MEDICAL EXAMINER Date Nov 9th 1908...Stage advanced Remarks runs fever" and this section is signed "C. D. Spivak" [sic]. The back-middle section of the application reads, "Application No. 957 Case No. 675 APPLICATION OF Ester Sayetta Address 1331 Opceola St., Received [Nov.] 9, 1908, Accepted Dec. 28, 1908, Admitted Jan. 1, 1909, Died Feb. 13 1909, Remarks Nov 12/08 Mrs Lorber to investigate" [sic].
