Abstract

1st JCRS Application for Admittance filled out in ink for Patient #951 J. Bilovsky "[t]o the Executive Committee of the Jewish Consumptives' Relief Society, Denver, Colorado...Received January 10 1908" [sic]. The application includes patient's name, sex, age, occupation, residential information, medical information, marital status [single], number and ages of children [none], and nearest relatives or friends; "(sister) Mrs. Annie Neshman 89 Johnson Av. Bklyn" and the application is signed Jacob Bilovsky [sic]. The bottom third of the application reads "RECOMMENDATION" and includes two names and addresses: Louis Schapiro [2709 Colfax] and B. Gridenberg [2779 W. Colfax]. The back of the application reads "REPORT OF MEDICAL EXAMINER" and this section has been left blank. The back-middle section of the application reads, "Application No. 676 Case No. 485 APPLICATION OF Jacob Bilovsky...Accepted Mar. 8 1908, Admitted Mar. 18, 1908, Discharged [Left] June 15 1908" [sic].
