Overview

Application for admittance to JCRS filled out by hand in ink for Abraham Kaiser. Information on the form includes his age, birth place, occupation, residence, marital status, children's ages, nearest relatives, and signed by Abraham Kaiser. Under "Recommendation" are two names and addresses, one name is illegible, the address reads 2708 W. Colfax and Louis Shapiro, 2709 W. Colfax. The back of the form has section entitled "Report of Medical Examiner," which is dated July 24, 1908 and states that the applicant is suffering from the 1st stage of pulmonary tuberculosis. Remarks reads "R. Apex bronch. [vesic] [breath] + moisture. L. apex, moisture less, Prog: Good for recovery" and is signed Emanuel Friedman. The middle section of the back is a form filled out by hand in ink. It reads "Application No. 853 Case No. 633, Application of, Abraham Kaiser, Received July 24, 1908, Accepted Oct. [7], 1908, Admitted Oct. 9, 1908, Discharged Dec. [19], 1908." The bottom of the page reads "Admission and Dismission Commitee" which is blank.

Inscriptions and Marks

Under "Remarks" is a note written in pencil which reads "Written Mrs. Lorber 8/12, Levin to input 9/14/08, 9/19/08 wrote Levin change of address, 9/24/08 Levin reported he could not find him at new address."
