Aaron Agreen's Application for Admittance to JCRS, 1908 July 23
Application for admittance to JCRS filled out by hand in ink for Aaron Agreen. Information on the form includes his age, birth place, occupation, residence, marital status, children's ages, nearest relatives, and signed by Aaron Agreen. The back of the form has section entitled "Report of Medical Examiner," which is dated July 24, 1908 and states that the applicant is suffering from the arrest [illegible] stage of pulmonary tuberculosis. Remarks read "Sclerosis (bronchial breathing) upper Right. mosture base R. [post] only occasionally and then after coughing only" and is signed Emanuel Friedman. The middle section of the back is a form filled out by hand in ink. It reads "Application No. 854 Case No. 607, Application of, Aaron Agreen, 2816 [S.] 14th Ave., Received July 24, 1908, Accepted Sept. 2, 1908, Admitted Sept. 4, 1908, Left Oct. 6, 1908." The bottom of the page reads "Admission and Dismission Commitee" which is blank.
