Application for admittance to JCRS filled out in ink for Abraham Fox. Information on the form includes his age, birth place, occupation, residence, marital status, children's ages, nearest friends, and signed by Abraham Fox. At the bottom of the front page, the section "Recommendation" is blank. The back of the form has section entitled "Report of Medical Examiner," which states the exam took place on January 4 1908, that Abraham Fox suffers from Stage II tuberculosis, with "involvement of right upper lobe and left apex" and signed by Hillkowitz. The middle section of the back is a form filled out by hand in ink. It reads "Application No.1010, Case No.712, Application of Abraham Fox, Address 1362 Grove St. c/o Mr. Goodman, Received Jan 2 1909, Accepted Mar 12 1909, Admitted March 12 1909, Left Apr 23 1909." There is a handwritten note in pencil that reads "Remarks 3/1-09 I was told by several people that Mr. Fox is also [illegible] pennyles[sic] and the people where he stays are also [illegible] being able to keep him [illegible] ti be admitted at once" signed by Disraelly. The bottom of the page reads "Admission and Dismission Committee" which is blank.
