Scope and Contents
From the Series: This series of patient records includes medical reports and histories, statistical data of patients, x-rays, and death certificates. The patient files include applications, correspondence and some may also contain photographs and personal items.
Physical Characteristics and Technical Requirements
On the address side of the envelope is a postal stamp in black ink which reads "St. Louis, MO., Feb. 10 11:30 1909." There is another postal stamp in black ink that reads "Denver, Colo, Feb 6 9:30pm 1909." On the front is also a stamp in red ink that shows a hand with a finger pointing towards the sender address that reads "return to writer [illegible]." On the back side of the envelope are two postal stamps. One, in black ink, upside down reads "Denver, Colo., Feb 11 7:30pm 1909." The second, in purple ink reads "[illegible] No. 41 Record, Feb 9 1909." Stamped in purple ink upside down on the back of the envelope a note reads "No Such Number." Envelope is ripped from top to bottom on the lefthand side.
