Lillian Robinson's Application for Admittance to JCRS, 1909 January 8
Application for admittance to JCRS filled out by hand in ink for Lillian Robinson. Information on the form includes her age, birth place, residence, occupation, marital status, children's ages, membership to beneficials societies or lodges, and nearest relatives. At the bottom of the page, under "Recommendation," is one name and address. The name is illegible but the address reads "1471 Knox Court." The back of the form has section entitled "Report of Medical Examiner," which states that the examination was performed January 9th, 1909 and that the patient suffers from Stage 2 tuberculosis. Remarks read "The left lung is practically [illegible]" and is signed by C.G. Hickey. The middle section of the back is a form filled out by hand in ink. It reads "Application No.1014, Case No.720, Application of Lillian Robinson, Address 1471 Knox Court, Received Jan 8 1909, Accepted Mar 20 1909, Admitted Mar 22 1909, Discharged Aug 9 1909." The bottom of the page reads "Admission and Dismission Committee" which is blank.
