Letter from the Pennsylvania Department of Health, 1909 January 2
Handwritten letter in black ink on Pennsylvania Department of Health letterhead addressed to "To whom it may concern" from "[illegible] B. Shepard [illegible]" in Allegheny county in Pittsburg, Pennsylvania. Letter informs that "[illegible] B. Shepard [illegible]" certifies that Harry Goldenson is a patient under his care for tuberculosis and Mr. Goldenson has been sick since February 06 and had a small hemorrhage May 08 which occurred again one week later. Informs that Mr. Goldenson had been under his care since November 24, "weight was then 102 [illegible] PM 101 2/5, Pulse 128, respiration 30. Present weight 100." Letter informs that the disease is advanced to a moderate degree and the patient "also has [illegible] and [illegible] of the vocal cords." Letter is signed [illegible] B. Shepard [illegible] followed by an illegible address in Pittsburg, Pennsylvania.
