Scope and Contents
From the Series: This series of patient records includes medical reports and histories, statistical data of patients, x-rays, and death certificates. The patient files include applications, correspondence and some may also contain photographs and personal items.
Physical Characteristics and Technical Requirements
On the address side of the envelope is a postal stamp in black ink which reads "Denver, Colo. March 24 3PM 1909." Next to the postal stamp is a stamp in the shape of a hand with a finger pointing to the sender address and reads "Returned to Writer, Unclaimed Denver, CO. Fee claimed at Denver, Colo." in purple ink. On the left side of the envelope are several notes. The first is written in pencil and reads "os J93" and the second also in pencil and circled reads "3/29." Below that is a note in black ink reads "6417" underlined and "3-24" underneath. On the far left side of envelope is a stamp in blue ink that reads "VP" and is underlined. Two other notes are written but are inelligible. On the back side of the envelope is a handwritten note in pencil on the far left side that reads "Unable to Deliver. McGovern" and is underlined.
