Application for admittance to JCRS filled out by hand in ink for Evelyn Lichtenstein. Information on the form includes her age, place of birth, occupation, residence, marital status, and is signed by Evelyn Lichtenstein. At the bottom of the page is a section entitled "Recommendation" that has one name and address that reads "Mrs. Schuman, 2115 Larimer St." The back of the form has a section entitled "Report of Medical Examiner," which states the exam took place on March 18, 1909 and reports that Evelyn Lichtenstein suffers from stage three tuberculosis. Also reports she had a temperature of 100 at 3pm and "prog: bad" and the report is signed by Emanuel Friedman. The middle section of the back is a form filled out by hand in ink. It reads "Application No. 1053, Case No. 734, Application of Evelyn Lichtenstein, Address 2221 Downing Ave, Received Mar 15 1909, Accepted Apr 20 1909, Admitted Apr 22 1909, Left Dec 20 1909." The bottom of the page reads "Admission and Dismission Committee" which is blank.
