Application for admittance to JCRS filled out by hand in ink for Fannie Greenberg. Information on the form includes her age, place of birth, occupation, residence, marital status, previous hospital stays, nearest relatives, and signed by Fannie Greenberg. At the bottom of the page is a section entitled "Recommendation" which is left blank. The back of the form has section entitled "Report of Medical Examiner," which states that the examination was conducted March 26, 1909 and the results were that the patient suffers from stage three tuberculosis. Report is signed by Emanuel Friedman. The middle section of the back is a form filled out by hand in ink. It reads "Application No. 1061, Case No. 748, Application of Fannie Greenberg, Address 1471 Knox Court, Received Mar 25 1909, Accepted May 4 1909, Admitted May 6 1909, Discharged April 30 1911." The bottom of the page reads "Admission and Dismission Committee" which is blank.
