Second Application for admittance to JCRS filled out by hand in ink for Abe Gutnick. Information on the form includes his age, place of birth, occupation, residence, marital status, previous hospital stays, nearest relatives, and is signed by Abe Gutnick. At the bottom of the page, under "Recommendation," are two names and addresses. The first reads "Mr. Isaacson, 1513 Larimer St." The second reads "M. [illegible], 1607 Larimer St." The back of the form has section entitled "Report of Medical Examiner" which is left blank. The middle section of the back is a form filled out by hand in ink. It reads "Application No. 421, Case No. 423, 2nd application of Abe Gutnick, Address 1517 Dale Court, Denver, Colorado, Received February 18 1907, Accepted Dec. 8 1907, Admitted Dec. 8 1907, Left January 4 1908." At the bottom of the middle section on the back, under "Remarks," are two notes that read "Disraelly and Mr. Lorber visited but he moved away [illegible] 13/07," and "Extend till Feb. 8th 1908, 1/2/08." The bottom of the page reads "Admission and Dismission Committee" which is blank.
