Jacob Greenberg's Application for Admittance to JCRS, 1909 April 19
Application for admittance to JCRS filled out by hand in ink for Jacob Greenberg. Information on the form includes his age, place of birth, occupation, residence, marital status, children's ages, membership to beneficial societies or lodges, nearest relatives, and is signed in Yiddish. At the bottom of the page is a section entitled "Recommendation" which is left blank except for a note that reads "Letter from Jacob Schoen." Back of the form has section entitled "Report of Medical Examiner" which states the exam took place on April 19 1909 and found the patient's stage of tuberculosis to be "doubtful." Medical examiner's remarks are illegible and it is signed by Emanuel Friedman. The middle section of the back is a form filled out by hand in ink. It reads "Application No. 1083, Case No. 750, Application of Jacob Greenberg, Address 1361 11th St., Received Apr 19 1909, Accepted May 4 1909, Admitted May 7 1909, Left Oct 5 1909." The bottom of the page reads "Admission and Dismission Committee" which is blank.
