Scope and Contents
From the Series: This series of patient records includes medical reports and histories, statistical data of patients, x-rays, and death certificates. The patient files include applications, correspondence and some may also contain photographs and personal items.
Physical Characteristics and Technical Requirements
On the front of envelope is a postal stamp in black ink that reads "North Station." A second postal stamp on the front of the envelope in black ink reads "Providence R.I. Apr 6, 11am, 1910." A note is scrawled in pencil across the front of the envelope that reads "find out who is Mr. Levin [illegible] patient from Providence."
