Harry Benjamin's Application for Admittance, 1909 May 27
Application for admittance to JCRS filled out by hand in ink for Harry Benjamin. Information on the form includes his age, birth place, occupation, residence, marital status, previous hospital stays, nearest relatives, and signed by Harry Benjamin. At the bottom of the page, under "Recommendation," is a note that reads "card from Dr. H. Abramowitz." The back of the form has section entitled "Report of Medical Examiner," filled out by hand in ink and signed by Robert L. Charles and states that the examination took place May 27, 1909. Report states that "occasional night sweats; lost about 15 lbs; regained 10 lbs. [illegible] in upper right lung; [illegible]; report from Dr. Lafluer (Montreal) says [illegible] bacilli found in sputum." The middle section of the back is a form filled out by hand in ink. It reads "Application No.1120, Case No.795, Application of Harry Benjamin, Address 1471 Knox Court, Received May 27 1909, Accepted May 28 1909, Readmitted July 12 1909, Died April 16 1910." The bottom of the page reads "Admission and Dimission Committee" which is blank.
