Application for admission to JCRS filled out by hand in ink for Alexander Echowitz. Information on the form includes his age, birth place, occupation, residence, marital status, children's ages, and nearest relatives. Form is signed "A Eckowitz." Nearest relative is listed as "Ethel Echowitz (wife) 2645 1/2 E. Mission St., Frisco." In the Recommendation section at the bottom of the page there is a note that reads "Letter from Rabbi M.S. Levy." The section "Report of the Medical Examiner" on the back side of the form states examination took place on October 18, 1909, found that Mr. Echowitz suffered from second stage tuberculosis and is signed "H.N. Drohn." The middle section of the back of the form reads "Application No. 1249. Case No. 871. Application of Alexander Echowitz. Address 1471 Knox. Ct. Received Oct 18 1909. Admitted Nov 16 1909. Discharged April 27/1910." The "Admission and Dismission Committee" section of the back of the form is left blank.
