Jake Finkelstein's Application for Admittance to JCRS, 1910 July 8-1911 August 17
JCRS application filled out by hand in ink for Jake Finkelstein. Information on the form includes his age, birth place, occupation, marital status, previous hospital stays, nearest relatives, and signed by Joseph Greenfield. At the bottom of the page, under "Recommendation" are two names both of which are illegible. The back of a form, "Report of Medical Examiner" informs that the examination took place on July 9, 1910 and reports that the patient suffers from advanced stage of tuberculosis and is signed by Dr. M. Krohn. The middle section of the back is a form filled out by hand in ink. It reads "Application No. 1507, Case No. 1030, Application of Jake Finkelstein, Address 1445 Hazel Court Denver Colo., Received July 8 1910, Admitted July 8 1910, Discharged Aug 17 1911." The bottom of the page reads "Admission and Dismission Committee" which is blank.
