Application for admission to JCRS filled out by hand in ink for Hyman Novack. Information on the form includes his age, birth place, occupation, residence, marital status, children's ages, nearest relatives, and signed by Hyman Novack. At the bottom of the page, the section "Recommendation," is left blank. The back of the form has section entitled "Report of Medical Examiner," which states that the examination took place July 26, 1910 and found Novack to be suffering from incipient pulmonary tuberculosis. Section is signed by Dr. M. Krohn. The middle section of the back is a form filled out by hand in ink. It reads "Application No.1523, Case No.1074, Application of Hyman Novack, Address 1522 Julian St Denver Colo., Received 7-25-10, Admitted Sept 7/10, Discharged May 2/11." The bottom of the page has a section titled "Admission and Dismission Committee" which is blank.
