Application of admittance to JCRS handwritten in pen for Abraham Goldstein. It has information such as age, place of birth (Russia), and marital status. In "To what beneficial societies or lodges do you belong", "none" is crossed out followed by "father member of OBA". For his nearest relatives, father and mother are written. On the back side, it is written he is on stage III of pulmonaty tuberculosis, and "consolidation in both lungs", by December 4, 1909. His application was received on December 1. 1909, he was admitted on May 17, 1910, and he passed away on July 22, 1910.
