Handwritten application in ink for admittance to the JCRS for Harry Goldberg. It includes information such as his age (20), his place of birth (Poland), and his occupation (barber). He is single and he has been to a sanatorium before. His nearest relative is his brother, Sam Goldberg, in New York City. On the backside, it is written he had tuberculosis stage III, and he had involvement on both lungs, the left being the worse. He was admitted on February 21, 1910, and he left on May 20, 1910.
