Overview

Application form of Emil Jusskowitz for admission as a patient to the Jewish Consumptives' Relief Society. He was age 40 at the time of the application. He immigrated to the U.S. from Hungary in 1880. He lived in New York City, New York for 30 years where he contracted tuberculosis. He had also been sick for 1 year upon arrival to Denver, Colorado. He was married and had two children. He belonged to the Franz Deak Lodge #11 I.O.B.A. The verso of the application states he was admitted on April 7, 1910 and died on July 30, 1910. There is a red stamp in the middle of the verso of the application that says "EMERGENCY" indicating his admission as an emergency case.
