Application form of Rachel Goodman for admission as a patient to the Jewish Consumptives' Relief Society. She was age 25 at the time of this application. She was born in Russia and immigrated to the United States in 1905. She lived in Boston Massachusetts when she contracted tuberculosis. She had been sick for 2 years upon arrival to Denver, Colorado. She was also single. The verso of the application is blank.
