Overview

Application form of Dave Kirstein for admission as a patient to the Jewish Consumptives' Relief Society. He was age 26 at the time of the application. He was born in Romania and immigrated to the United States in 1906. He lived in Selma, Alabama when he contracted tuberculosis. He had been sick for 5 months upon arrival to Denver, Colorado. He was married and had one child. He also worked as an clerk. The verso of the application states he was admitted on May 16, 1910 and left on August 27, 1910.
