Application form of Nathan Leib for admission as a patient to the Jewish Consumptives' Relief Society. He was age 24 at the time of the application. He was born in Austria and immigrated to the United States in 1905. He lived in New York when he contracted tuberculosis. He had been sick for 3 years upon arrival to Denver, Colorado. He also worked as a leather cutter. The verso of the application states he was admitted as an emergency case on May 16, 1910 and died on June 15, 1910.
