Max Cohen's Application for Admission to JCRS, 1910 June 14
Second application form of Max Cohen for admission as a patient to the Jewish Consumptives' Relief Society. He was age 22 at the time of this application. He was born in Szack, Russia and immigrated to the United States in 1907. According to this application he lived in New York when he contracted tuberculosis. He states he has been sick for five months. He also worked as an operator. The verso of the application states he was admitted as an emergency patient on June 14, 1910 and left on September 14, 1910.
