Handwritten application in pencil for admittance to the JCRS for Harry Cinicove. It includes information such as age (43), place of birth (Russia), and occupation (tinsmith). He was married and and had six children. He was in a sanatorium in Pennsylvania, had no means of support, and his nearest relative was his brother-in-law in Philadelphia, PA. On the backside it is written he had Tuberculosis Stage II, that he was admitted on September 8, 1910 and he died on September 12, 1910.
