Max Perlman's Application for Admission to JCRS, 1908 February 22
Application form of Max Perlman for admission as a patient to the Jewish Consumptives' Relief Society. He was age 25 at the time of the application. He was born in Russia and immigrated to the United States in 1906. He lived in Boston when he contracted tuberculosis. He had been sick for a year and half upon arrival to Denver, Colorado. His occupation states he worked as a salesman. He was in the National Jewish Hospital before applying for JCRS. The verso of the application states the application was received on February 22, 1908. There is a note on the bottom of the page that states Perlman left for New Mexico to look for work in April, 1908.
