Max Perlman's Application for Admission to JCRS, 1908 October 18
Application form of Max Perlman for admission as a patient to the Jewish Consumptives' Relief Society. He was age 26 at the time of the application. He was born in Russia and immigrated to the United States in 1906. He lived in Boston, but contracted tuberculosis in Philadelphia. He had been sick for two years upon arrival to Denver, Colorado. His occupation states he worked as a salesman. He belonged to the Denver Benevolent Society. He was in the National Jewish Hospital before applying for JCRS. The verso of the application states he was admitted on October 18th, 1908 as an emergency patient, and was discharged on October 28th, 1908.
