Overview
Application form of Max Perlman for admission as a patient to the Jewish Consumptives' Relief Society. He was age 26 at the time of the application. He was born in Russia and immigrated to the United States in 1906. He lived in Boston, but contracted tuberculosis in Philadelphia. He had been sick for two and a half years upon arrival to Denver, Colorado. His occupation states he worked as a salesman. He belonged to the Denver Benevolent Society. He was admitted to JCRS twice before. The verso of the application states he was admitted on March 18th, 1909 and was discharged on August 2nd, 1909.
