Max Perlman's Application for Admission to JCRS, 1911 January 30
Application form of Max Perlman for admission as a patient to the Jewish Consumptives' Relief Society. He was age 28 at the time of the application. He was born in Russia and immigrated to the United States in 1906. He lived in Boston, but contracted tuberculosis in Philadelphia. He had been sick for five years since his arrival to Denver, Colorado. His occupation states he worked as a clerk. He was admitted to JCRS three times before. The verso of the application states he was admitted on May 2nd, 1911 and died on October 16, 1912.
