Overview

Application form of Sam Fisher for admission as a patient to the Jewish Consumptives' Relief Society. He was age 25 at the time of the application. He was born in Russia and immigrated to the United States in 1906. He lived in Chicago when he contracted tuberculosis. He had been sick for two years upon his arrival to Denver, Colorado. His occupation states he worked as an operator. The verso of the application states he filed an application on April 10, 1911, but his admission letter was returned to JCRS.
