Leo Frankel's second handwritten application for admittance to JCRS. Includes information such as age (37), place of birth (Russia), and occupation (clerk). He was married, has 4 children, had been to JCRS sanatorium before, and his neares relatives were his wife and brother, in Philadelphia. On the backside, it reads he was Admitted on January 26, 1911, and left on November 26, 1911. "Left without notice because Dr. Schwatt found it necessary to move him from the pavilion to a tent to make room for a patient who needed it more than him."
